Research
- Energy: Selective strength signals do not establish everyday energy or reliable sprint-power benefit.
REALITY CHECK // A 2024 synthesis of 17 studies (317 participants; 21% women) found selective maximal-strength improvement: SMD 0.47 (95% CI 0.04-0.89), with a lower-body signal and no clear upper-body benefit. Body-composition and anaerobic-power findings are inconsistent. Homocysteine lowering is a marker change, not proof of fewer cardiovascular events. High-dose chronic use can raise total cholesterol.
Levels: how we grade evidence. Grey = evidence of no benefit; amber = evidence it makes this goal worse.
Ratings are self-reported by a self-selected group of users: how it felt, not proof that it works.

How it was studied
UPPER LIMIT // No formal EFSA/NIH UL is supplied. A synthesis at at least 4 g/day for 6-24 weeks found total cholesterol +0.34 mmol/L (95% CI 0.02-0.65), without significant LDL/HDL/triglyceride differences in that analysis. A health-claim threshold is not a safety ceiling.

Mechanism
Betaine is an osmolyte and methyl donor in homocysteine remethylation. Plausible mechanisms do not establish muscle growth or cardiovascular prevention. Betaine HCl also adds acid and has a different clinical context.
Typical studied dose
Exercise trials typically used 2.5 g/day anhydrous betaine for at least 7 days. Homocysteine/liver research used different, often higher exposures around 3-6 g/day. Remethylation-disorder treatment requires specialist care. These are study descriptions rather than personal regimens.

Forms and label traps
| Form on label | Active content | Studied range / day | Absorption & GI tolerance | Verdict |
|---|---|---|---|---|
| Anhydrous betaine / trimethylglycine Best for: Matching anhydrous exercise-study exposure. |
100% elemental / active | 2500–2500 mg | Readily absorbed; standard exercise material. High quantities can cause nausea, diarrhea or fishy odor. |
Studied standard [CHECK // MATERIAL_AND_LABEL] Declared anhydrous ingredient is 100% betaine. The typical exercise-study amount was 2.5 g/day. |
| Betaine hydrochloride (HCl) Best for: Clinician-directed gastric-acid assessment. |
75% elemental / active | no comparable trial range | Provides betaine plus acid. May aggravate ulcers or reflux. |
Caution [CHECK // MATERIAL_AND_LABEL] Approximately 75% betaine by supplied stoichiometry. Content conversion does not prove TMG exercise equivalence; the trial-comparison band is disabled. |
| Food-derived betaine (beetroot/spinach/bran) Best for: Dietary intake rather than a numeric trial match. |
Unstandardized: active content unknown | no comparable trial range | Bioavailable but variable concentration. Depends on food matrix and tolerance. |
Caution [CHECK // MATERIAL_AND_LABEL] No assay is supplied; beetroot powder mass cannot be converted confidently into betaine. |
Common label traps
- TRAP_01 // TMG and HCl are interchangeable
Different composition and clinical context prevent transferring exercise or gastric-acid claims between forms. - TRAP_02 // Lower homocysteine proves heart protection
Marker reduction does not establish fewer cardiovascular events. High chronic exposures can raise total cholesterol. - TRAP_03 // It is a proven testosterone muscle-builder
The hormone signal comes from one crossover study; performance and body-composition findings are inconsistent.

Reader questions
Is TMG the same as betaine HCl?
No. Anhydrous TMG is used in most exercise trials; HCl contains about 75% betaine plus acid and has different tolerability.
Can it raise cholesterol?
At at least 4 g/day for 6-24 weeks a synthesis found total cholesterol about 0.34 mmol/L higher. This differs from typical 2.5 g/day exercise protocols.
Does homocysteine lowering prove longevity?
No. A biochemical marker effect does not establish longer life or fewer cardiovascular events.
Stack interactions
- Caution PPIs and H2 blockers [CAUTION // CONTEXT_MATTERS]
HCl changes gastric acidity. A dog drug-absorption experiment cannot establish clinical magnitude; review pH-sensitive medication with the clinician.
PMID 23980865 PMID 38474790 - Caution Creatine and other methyl donors [CAUTION // CONTEXT_MATTERS]
Mechanistic overlap does not prove incremental benefit. Overlapping powders can obscure total intake; no indexed combination trial establishes synergy here.
Red flags: interactions and who should avoid it
Interactions
HCl changes gastric acidity and may conflict with PPIs/H2 blockers or alter pH-sensitive drug absorption; its indexed pharmacokinetic experiment used dogs. A significant glucose-drug interaction is not established. Methyl-donor overlap does not establish useful stack synergy.
Avoid or ask a doctor first if
Supplement-dose pregnancy/breastfeeding safety is insufficient. Homocystinuria/remethylation disorders require specialist monitoring; severe complications are described with therapeutic exposures. Ulcers/reflux/acid suppression warrant particular caution with HCl. Dyslipidemia requires review before chronic high-dose use. Nausea, diarrhea and fishy odor can occur.
