Folate / Folic Acid
Also sold as: Vitamin B9 · Folic acid · L-methylfolate · 5-MTHF
Folate supports one-carbon metabolism and DNA synthesis. Folic acid has established periconceptional evidence for preventing neural tube defects; that indication does not establish general cardiovascular or cognitive enhancement.

Research
- Focus: Trials and reviews in cognitively healthy middle-aged and older adults generally find no meaningful cognitive benefit; high-homocysteine subgroups remain uncertain.
- Longevity: Lower homocysteine has not consistently translated into fewer myocardial infarctions or lower overall or cardiovascular mortality.
REALITY CHECK // MTHFR variants do not make all folic acid useless. Folic acid has the strongest hard-endpoint pregnancy record, whereas short-term methylfolate bioavailability does not establish superior prevention of birth defects. UMFA concerns and cancer hypotheses require context, not blanket claims of harm.
Levels: how we grade evidence. Grey = evidence of no benefit; amber = evidence it makes this goal worse.
Ratings are self-reported by a self-selected group of users: how it felt, not proof that it works.

How it was studied
UPPER LIMIT // The dossier cites a 1 mg/day adult limit for synthetic folic acid from supplements and fortified food, excluding natural food folate. Current jurisdiction and form-specific applicability need primary-document review; numeric calculator ULs remain unset to avoid extending the folic-acid limit to every folate salt.
Summary
The clearest clinical benefit is prevention of fetal neural tube defects around conception. Homocysteine falls reliably, but it is a surrogate, and mortality and coronary outcomes do not follow it. Methylfolate augmentation of depression treatment is a clinician-led, mixed-evidence indication.
Mechanism
Reduced folates carry one-carbon units for nucleotide synthesis and methylation. Folic acid requires reduction and conversion; 5-MTHF bypasses DHFR/MTHFR steps, while B12 status remains important for safe interpretation of anemia and neurological symptoms.
Typical studied dose
Periconceptional studies/guidance in the dossier use 400-800 micrograms/day (0.4-0.8 mg), from at least four weeks before conception through the first 12 weeks of pregnancy. Psychiatric methylfolate discussions involve 7.5-15 mg, a separate medical use rather than a routine nutrient target.

Forms and label traps
| Form on label | Active content | Studied range / day | Absorption & GI tolerance | Verdict |
|---|---|---|---|---|
| Folic acid, declared nutrient mass Best for: Periconceptional evidence base. |
Extract weight · ~100% marker | 0.4–0.8 mg | Stable synthetic form; requires metabolic conversion. Usually well tolerated. |
Studied standard [FORM // REFERENCE] Enter mg of folic acid, not micrograms or dietary folate equivalents; 400 micrograms = 0.4 mg. No absorption multiplier is applied. |
| L-5-MTHF / L-methylfolate, declared nutrient mass Best for: Alternative folate form when clinically appropriate. |
Unstandardized: active content unknown | no comparable trial range | Short-term bioavailability was equivalent to folic acid in men in the cited trial. Usually tolerated; agitation reports at high doses are not a defined syndrome. |
Good option [FORM // CHECK_BASIS] No salt-to-nutrient conversion or routine dose equivalence is established here. Psychiatric milligram regimens are separate from nutritional microgram use. |
| Folinic acid / leucovorin Best for: Specific clinician-directed folate therapy. |
Unstandardized: active content unknown | no comparable trial range | Reduced folate with intracellular conversion. Generally tolerated; context is medical. |
Good option [FORM // CHECK_BASIS] Not interchangeable with folic acid or 5-MTHF on the basis of the bottle mass. Unknown comparable clinical interval is deliberately unavailable. |
Common label traps
- TRAP_01 // MTHFR means folic acid never works
A common variant does not establish inability to use all folic acid, nor superiority of methylfolate for neural tube defect prevention. - TRAP_02 // Lower homocysteine means a longer life
The biochemical reduction is reliable, but trials do not consistently show fewer coronary events or deaths. - TRAP_03 // Folic acid simply causes cancer
Dietary associations and supplementation RCTs differ. The dossier reports no overall CRC increase in the cited trial meta-analysis; lesion timing and chronic megadoses remain cautions.
Other outcomes studied

- NEURAL TUBE DEFECT PREVENTION Strong · benefit
Periconceptional folate review: relative risk about 0.31, roughly 69% relative reduction. This pregnancy endpoint must not be mapped to anxiety.
PMID 26662928 PMID 37526714 - HOMOCYSTEINE Strong · benefit
Randomized-trial meta-analysis reports roughly 25% reduction, influenced by baseline status. This is a surrogate, not proof of cardiac protection.
PMID 11339443 PMID 25323814 - STROKE Limited · benefit
Meta-analysis reports RR 0.82, with stronger signals in unfortified populations and longer trials; this does not establish improved everyday focus.
PMID 17544768 PMID 29078842 - DEPRESSION AUGMENTATION Limited · benefit
Evidence is mixed: FolATED was negative, while reviews suggest a modest, uncertain signal for methylfolate in selected patients. This is psychiatric treatment augmentation.
PMID 25052890 PMID 29442609 - COLORECTAL CANCER Limited · no benefit
Dietary folate associations are observational. Supplementation trials do not establish CRC prevention or an overall increase in the cited meta-analysis; high-dose lesion-progression hypotheses remain uncertain.
PMID 19863600 PMID 41314820 - PREECLAMPSIA Moderate · no benefit
High-dose supplementation did not prevent preeclampsia in the dossier evidence; this does not negate periconceptional neural tube prevention.
PMID 29882975 - MIGRAINE Limited · benefit
Small, low-quality B6/folate/B12 studies suggest benefit in selected groups, often with MTHFR variants; the effect cannot be isolated to folate.
PMID 32279306 - FOLATE-DEFICIENCY ANEMIA Moderate · benefit
Correction of confirmed folate-deficiency megaloblastic anemia is established clinical practice; no dedicated numerical trial citation is supplied. Exclude concurrent B12 deficiency.
Reader questions
Which form should I choose if I have an MTHFR variant?
A variant alone does not justify replacing every folic-acid product or using high-dose methylfolate. Folic acid has pregnancy hard-endpoint evidence, and short-term absorption comparisons do not establish universal clinical superiority.
Why do some methylfolate products contain 15 mg?
That is a psychiatric augmentation scale, not a routine nutritional dose. Discuss symptoms, antidepressants and B12 status with the treating clinician rather than escalating from a microgram prenatal dose.
Is folic acid appropriate in pregnancy?
The dossier supports 400-800 micrograms/day around conception. Previous neural tube defects and certain medicines change care needs; arrange individualized prenatal advice.
What if methylfolate makes me anxious or irritable?
Do not assume a proven overmethylation diagnosis. Stop escalating and review dose, medicines and symptoms with a clinician, particularly with psychiatric treatment.
Stack interactions
- Caution Vitamin B12, metformin and proton pump inhibitors [CAUTION // REVIEW_CONTEXT]
Folate can mask the hematological presentation of B12 deficiency. Metformin and long-term acid suppression can worsen B12 status; evaluate B12 rather than relying on a normal blood count.
PMID 38987872 - Caution Methotrexate and folate antagonists [CAUTION // REVIEW_CONTEXT]
Folic acid may reduce rheumatology methotrexate toxicity, whereas folinic-acid rescue in oncology is a separate regimen. Trimethoprim, sulfonamides and sulfasalazine alter folate handling. The dossier lacks dedicated interaction trials; prescribing teams determine use. - Caution Antidepressants and anticonvulsants [CAUTION // REVIEW_CONTEXT]
Methylfolate augmentation has mixed clinical evidence and belongs with the treating clinician. Phenytoin and other anticonvulsants require review because interactions can be bidirectional.
PMID 24524097 - Caution Riboflavin and B6 [CAUTION // REVIEW_CONTEXT]
These are pathway cofactors. Their biochemical role does not prove additional clinical benefit from high-dose stacking without deficiency.

Their biochemical role does not prove additional clinical benefit from high-dose stacking without deficiency.
Avoid or ask a doctor first if
Assess untreated B12 deficiency before high-dose folate: blood counts can improve while B12-related neurological injury continues. Pregnancy is an indication for appropriate folate, not a contraindication. Prior neural tube defects, anticonvulsants, psychiatric dosing and existing neoplastic lesions require individualized care.
