Milk Thistle
Also sold as: Silymarin · Silybum marianum · Silibinin · Silybin
Milk-thistle seed extracts contain the silymarin flavonolignan complex. Trials focus on liver enzymes and metabolic biomarkers, which are different outcomes from liver regeneration, survival or antiviral treatment.

Research
- Longevity: Cochrane reviews in alcoholic and viral liver disease found no demonstrated mortality benefit; improved fatty-liver enzymes do not establish longer life.
REALITY CHECK // Lower enzymes in fatty-liver trials are not evidence that the liver has been detoxified or regenerated. Negative hepatitis-C and alcoholic/viral-liver-disease outcome data must remain visible alongside positive metabolic-marker findings. Tea, oral extract and intravenous silibinin are not interchangeable.
Levels: how we grade evidence. Grey = evidence of no benefit; amber = evidence it makes this goal worse.
Ratings are self-reported by a self-selected group of users: how it felt, not proof that it works.

How it was studied
UPPER LIMIT // No formal EFSA/NIH UL is documented for milk thistle or silymarin in this dossier. Trial tolerability cannot be converted into a maximum safe long-term dose.
Summary
NAFLD/NASH meta-analyses report lower ALT/AST, and diabetes meta-analyses report improvements in glycemic markers. These surrogate outcomes do not establish tissue regeneration, fibrosis reversal or survival benefit. Oral silymarin did not improve ALT or HCV RNA in the landmark trial of interferon nonresponders; intravenous silibinin for poisoning is a separate hospital treatment.
Mechanism
Silymarin contains flavonolignans, including silibinin, with proposed antioxidant, inflammatory and metabolic effects. Poor water solubility and rapid conjugation limit oral exposure; mechanistic plausibility does not establish clinical liver repair.
Typical studied dose
The supplied dossier describes oral trials using hundreds of mg of silymarin daily over weeks to months but does not give a reliable exact trial-by-form range. No exact calculator target is inferred. Extract mass, standardized silymarin content, silibinin content and raw-seed equivalents are distinct quantities.

Forms and label traps
| Form on label | Active content | Studied range / day | Absorption & GI tolerance | Verdict |
|---|---|---|---|---|
| Standardized seed extract, declared 70-80% silymarin Best for: Identifying a standardized oral preparation used in the clinical literature. |
Unstandardized: active content unknown | no comparable trial range | Low oral exposure due to poor solubility and conjugation; pharmacokinetics vary by liver disease. Usually mild nausea or loose stools; individual tolerability varies. |
Studied standard [REFERENCE // CHECK_PREPARATION] 80% is a stated example marker concentration. Exact extract-mass trial target is not supplied; calculator comparison is unavailable. |
| Silibinin-phosphatidylcholine complex (phytosome) Best for: Evaluating a product that explicitly declares complex and silibinin content. |
Unstandardized: active content unknown | no comparable trial range | Designed to increase oral exposure; exact fold advantage and superior clinical outcomes are not established here. Generally reported as well tolerated; comparative GI data are limited. |
Caution [CAUTION // CHECK_PREPARATION] No assumed 1:1 conversion, marker fraction or trial-equivalent dose; 0 denotes unknown composition. |
| Raw milk-thistle seed powder or tea Best for: Traditional food use rather than reproducing a standardized trial preparation. |
Unstandardized: active content unknown | no comparable trial range | Variable content; poorly water-soluble silymarin is inefficiently extracted into tea. Generally tolerated, but botanical allergy remains relevant. |
Caution [CAUTION // CHECK_PREPARATION] No verified raw-seed-to-extract conversion or therapeutic tea dose. |
Common label traps
- TRAP_01 // Detoxifies and regenerates the liver
ALT/AST reductions are surrogate changes. Histological repair, fibrosis reversal and improved survival have not been established by these findings. - TRAP_02 // Protects against drinking or cures hepatitis C
Oral silymarin failed on ALT/HCV RNA in a major HCV RCT. There is no demonstrated hangover-protection or alcohol-safety protocol here. - TRAP_03 // 8000 mg means a stronger extract
A raw-herb equivalent is not the weighed extract or its measured silymarin content. A ratio alone is not standardization. - TRAP_04 // A better-absorbed phytosome guarantees better outcomes
Greater exposure does not automatically establish superior clinical liver outcomes.
Other outcomes studied
Outside the five leaderboard goals. Same evidence levels as above.
- NAFLD/NASH liver enzymes Moderate · benefit
RCT meta-analyses report lower ALT/AST in adults with fatty liver. Exact pooled effect sizes are not extracted here, and enzyme improvement is not proven histological repair or better prognosis.
PMID 38579127 PMID 33418491 PMID 29245314 PMID 40738719 - Insulin resistance Moderate · benefit
An RCT meta-analysis reports improvements in fasting insulin/HOMA-IR; exact pooled magnitudes are unavailable and these remain metabolic surrogates.
PMID 39855603 - Type 2 diabetes glycemic markers Moderate · benefit
Meta-analyses report lower fasting glucose and HbA1c in adults with diabetes, with heterogeneous trials and no exact effect size supplied. Adjunctive use requires medication review.
PMID 27340676 PMID 30477860 - Lipids and combination products Limited · benefit
A metabolic signal exists for berberine-silymarin combinations. This does not identify the effect of silymarin alone.
PMID 30632209 - Chronic hepatitis B adjunct Limited · benefit
Combination-therapy meta-analysis suggests liver-marker changes, but limited trial quality prevents an antiviral-treatment claim.
PMID 23247631 - Chronic hepatitis C Limited · no benefit
Oral silymarin did not significantly reduce ALT or HCV RNA versus placebo in the landmark RCT in interferon nonresponders; earlier trials were inconclusive or negative.
PMID 22797645 PMID 16460486 - Alcoholic/viral liver disease mortality and histology Limited · no benefit
Cochrane reviews found no demonstrated improvement in mortality, complications or histology; apparent benefits depended on lower-quality studies.
PMID 17943794 PMID 16279916 - Amatoxin poisoning: hospital intravenous use Limited · benefit
Case series and a review of reported poisonings describe intravenous silibinin as adjunctive treatment. This is emergency hospital care, not evidence for an oral supplement.
PMID 36129244 PMID 23330770 PMID 6862461 - Drug-induced liver injury prevention Limited · benefit
Systematic/network reviews find an uncertain preventive signal, including anti-tuberculosis treatment. Chemoprevention remains unsettled and clinician-led.
PMID 26633044 PMID 33043729 - Acne and isotretinoin protection No established evidence
Relevant clinical claims lack indexed references in this dossier. Efficacy is unestablished, not disproved by a cited trial.
Reader questions
Which form should I compare on the label?
Look for weighed extract and measured silymarin content. Tea has low extraction, and a phytosome changes delivery without proving superior liver outcomes. Silymarin is a mixture; silibinin is a component.
Should I take it before drinking alcohol?
No protective regimen is established. Milk thistle does not make alcohol safe and should not be used to dismiss abnormal tests or symptoms.
Can it protect me from acetaminophen or mushroom toxicity?
Do not rely on an oral supplement. Suspected overdose or poisonous-mushroom ingestion requires urgent medical assessment; intravenous silibinin is a separate hospital intervention.
What if it causes nausea?
Food may help mild GI discomfort, but persistent or severe symptoms need assessment. Rash, swelling or breathing problems may indicate allergy.
Stack interactions
- Caution Diabetes medication [CAUTION // CLINICAL_REVIEW]
Human glucose-lowering evidence supports possible additive hypoglycemia. Review therapy and monitor glucose with the treating clinician.
PMID 27340676 PMID 30477860 - Caution Warfarin and immunosuppressants [CAUTION // CLINICAL_REVIEW]
CYP/P-glycoprotein effects are theoretical/in vitro in this dossier; clinical magnitude is uncertain, especially with narrow therapeutic windows. - Caution Berberine [COMBINATION // ATTRIBUTION_LIMIT]
Combination meta-analysis cannot allocate metabolic effects to milk thistle alone.
PMID 30632209 - Caution NAC or curcumin [CAUTION // CLINICAL_REVIEW]
Popular liver-support combinations have no established synergy in this source set.
Red flags: interactions and who should avoid it
Interactions
Glucose-lowering effects in type 2 diabetes may add to medication effects. CYP2C9/CYP3A4 and P-glycoprotein concerns for warfarin or immunosuppressants are theoretical/in vitro here, without a supplied clinical interaction study; review narrow-therapeutic-index drugs. Hormonal/estrogenic concerns are unverified clinical signals, not proven contraceptive failure.
Avoid or ask a doctor first if
Asteraceae allergy may cross-react and can be serious. Pregnancy, breastfeeding, children and hormone-sensitive conditions need individual review because data are inadequate. Nausea, bloating and loose stools can occur. Do not delay evaluation of jaundice, abnormal liver tests or suspected drug injury; milk thistle does not make alcohol, acetaminophen overdose or mushroom ingestion safe.
