Pantothenic Acid

Also sold as: Vitamin B5 · D-pantothenic acid · Calcium pantothenate · Pantethine

Vitamin B5 is a precursor of coenzyme A. Nutritional B5, lipid-focused pantethine and topical dexpanthenol have different uses and cannot share one clinical dose target.

Research

  • Energy: An older small exercise RCT was null; a role in coenzyme A does not demonstrate extra energy in people without deficiency.

REALITY CHECK // A coenzyme A mechanism does not make B5 an energy booster. The acne evidence is one small sponsored blend trial, while the lipid signal belongs to pantethine. Reports of hair loss, insomnia and agitation at megadoses remain anecdotes rather than established adverse-effect rates.

Levels: how we grade evidence. Grey = evidence of no benefit; amber = evidence it makes this goal worse.

Ratings are self-reported by a self-selected group of users: how it felt, not proof that it works.

How it was studied

01 // STUDIED_DOSE_RANGE
Form and population matter
The acne RCT used about 2.2 g/day of a pantothenic-acid-based blend for 12 weeks; this is not a verified pure-B5 mass. Pantethine trials used 600-900 mg/day for 16 weeks. Adult adequate intake around 5 mg/day is a nutritional reference, not an acne regimen.
02 // TIMING_&_MATRIX
Administration context
Meal timing and separation from biotin lack robust clinical evidence. Nutritional intake, pantethine therapy and topical creams are different contexts.
03 // EVALUATION_WINDOW
Match the studied window
Acne: 12 weeks in one small sponsored blend study. Lipids: 16 weeks in pantethine trials. Neither supports immediate extra energy.
04 // SAFETY_&_QUALITY_GATE
Review limits and medicines
No EFSA/IOM upper limit is established in the dossier because toxicity data are insufficient. The adult adequate intake near 5 mg/day is not a safety ceiling. Long-term 5-10 g/day acne megadoses have not been established safe. Avoid unsupervised megadoses during pregnancy or lactation; gastrointestinal upset and diarrhea can occur at high doses. Topical panthenol can rarely cause contact allergy. Persistent acne deserves appropriate clinical care.

UPPER LIMIT // No EFSA/IOM upper limit is established in the dossier because toxicity data are insufficient. The adult adequate intake near 5 mg/day is not a safety ceiling. Long-term 5-10 g/day acne megadoses have not been established safe.

Summary

Deficiency is very rare. A single small sponsored acne trial used a roughly 2.2 g/day B5-based blend, not pure B5. Pantethine has separate lipid RCTs; topical panthenol data do not support oral skin claims.

Mechanism

Pantothenate contributes to coenzyme A and acyl-carrier protein, enabling fatty-acid and energy metabolism. Essential biochemical function is not evidence that more substrate improves performance when intake is adequate.

Typical studied dose

The acne RCT used about 2.2 g/day of a pantothenic-acid-based blend for 12 weeks; this is not a verified pure-B5 mass. Pantethine trials used 600-900 mg/day for 16 weeks. Adult adequate intake around 5 mg/day is a nutritional reference, not an acne regimen.

Upper limit

No EFSA/IOM upper limit is established in the dossier because toxicity data are insufficient. The adult adequate intake near 5 mg/day is not a safety ceiling. Long-term 5-10 g/day acne megadoses have not been established safe.

Forms and label traps

Forms compared
Form on labelActive contentStudied range / dayAbsorption & GI toleranceVerdict
Calcium D-pantothenate, compound mass
Best for: Ordinary B5 replacement if needed.
92% elemental / active no comparable trial range Stable standard nutritional salt.
Very good at nutritional intakes.
Studied standard [FORM // REFERENCE]
About 92% pantothenic-acid composition by mass; use conversion only when the label actually lists salt mass, not already-declared vitamin B5. The acne blend does not establish a pure-B5 interval.
Pantethine
Best for: Form-specific lipid evidence.
Extract weight · marker not quantified 600–900 mg Different derivative studied for lipids.
Nausea or other GI symptoms can occur at higher doses.
Good option [FORM // CHECK_BASIS]
Compare full pantethine mass with its own trials, without converting into a B5 equivalent.
Free D-pantothenic acid
Best for: Declared vitamin mass.
Unstandardized: active content unknown no comparable trial range Absorbable, but hygroscopic and less stable than the calcium salt.
Generally good at ordinary nutrient intakes.
Good option [FORM // CHECK_BASIS]
No isolated pure-B5 acne dose interval can be inferred from the multi-ingredient blend. Unknown comparator intentionally unavailable.

Common label traps

  • TRAP_01 // 5-10 g B5 reliably clears acne
The sole small sponsored trial used a roughly 2.2 g B5-based blend. It neither isolates B5 nor establishes safety of much larger doses.
  • TRAP_02 // B5 causes hair loss by washing out biotin
Both use SMVT, but clinically important competition and hair loss in humans are poorly demonstrated.
  • TRAP_03 // Panthenol creams prove oral B5 hydrates skin
Topical barrier and wound findings cannot be transferred to swallowed B5.

Other outcomes studied

Outside the five leaderboard goals. Same evidence levels as above.

  • ACNE Limited · benefit
One sponsored RCT in 41 adults found fewer lesions after 12 weeks with a roughly 2.2 g/day B5-based blend; not proof for isolated B5 or 5-10 g megadoses.
    PMID 24831048
  • LIPIDS / PANTETHINE Moderate · benefit
Triple-blind placebo/diet-controlled studies at 600-900 mg/day reported about 11% LDL reduction over 16 weeks. This belongs to pantethine and does not replace indicated lipid care.
    PMID 24600231 PMID 21925346
  • TOPICAL SKIN BARRIER Limited · benefit
Narrative reviews describe topical dexpanthenol barrier, hydration and re-epithelialization findings in heterogeneous small studies; no oral inference.
    PMID 28503966 PMID 32610604 PMID 12113650
  • DEFICIENCY Limited · benefit
Historical induced-deficiency studies describe burning feet, fatigue and GI symptoms. Isolated deficiency is rare, so this does not justify routine megadoses.
    PMID 14361315 PMID 13587673
  • RHEUMATOID ARTHRITIS No established evidence
Old observations of altered B5 status do not establish causation or a supplementation treatment benefit. This is unestablished, not a negative treatment RCT.
    PMID 14056012 PMID 14404151

Reader questions

Does B5 cause hair loss through biotin competition?

There is a shared transporter, but convincing clinical proof for the hair-loss claim is lacking. Do not diagnose biotin deficiency from an internet mechanism.

What acne dose is both effective and safe?

No reliable isolated-B5 dose is established. The small trial used a blend and does not justify 5-10 g/day. A clinician can help choose evidence-based acne treatment.

Is pantethine the same as a B5 multivitamin?

No. Pantethine lipid studies used hundreds of milligrams, whereas ordinary B5 nutritional intake is around 5 mg/day. These are different compounds and endpoints.

Can I use megadoses indefinitely?

Long-term safety data are insufficient and there is no formal UL. Lack of a UL is not permission to increase indefinitely.

Stack interactions

  • Caution Biotin [CAUTION // REVIEW_CONTEXT]
The shared SMVT transporter creates theoretical competition at megadoses; clinical depletion or a proven hair-loss mechanism is not established.
  • Caution Lipid-lowering or glucose-lowering medicines [CAUTION // REVIEW_CONTEXT]
Pantethine may add to lipid effects; clinical interaction harm is not well documented. Ordinary pantothenic acid has few established drug interactions.
    PMID 24600231
  • Caution Antibiotics [CAUTION // REVIEW_CONTEXT]
The dossier identifies no documented clinically important B5-antibiotic interaction; absence of evidence is not a reason to invent a spacing protocol.
