Vitamin B6
Also sold as: Pyridoxine · Pyridoxal-5-phosphate · P5P
An essential enzyme cofactor used for deficiency correction and selected clinical indications. High-dose B6 is also marketed for nerves and anxiety, despite a real neuropathy hazard.

Research
- Anxiety: One short RCT in young adults found lower self-reported anxiety with 100 mg/day for about one month; this high dose is not a routine self-care recommendation.
REALITY CHECK // B6 is not a general nerve-repair drug: excess pyridoxine can produce the very sensory symptoms people hope to treat. P5P superiority and exemption from toxicity are not established in large head-to-head trials; the anxiety signal comes from one short high-dose study.
Levels: how we grade evidence. Grey = evidence of no benefit; amber = evidence it makes this goal worse.
Ratings are self-reported by a self-selected group of users: how it felt, not proof that it works.

How it was studied
UPPER LIMIT // The dossier cites the US adult UL of 100 mg/day total B6. Its EFSA 25 mg/day figure is historical (2000); the dossier flags a lower 2023 revision around 12 mg/day without verification. The EFSA calculator limit is therefore left unavailable pending primary-source review. ULs are not targets and do not authorize long-term high dosing.
Summary
Pregnancy nausea has better clinical support than anxiety. Older PMS trials are low quality, carpal tunnel results are unconvincing, and lowering homocysteine is not proof of cardiovascular protection. Excess B6 can itself cause sensory neuropathy.
Mechanism
Pyridoxal phosphate is a cofactor in amino-acid and neurotransmitter metabolism, including decarboxylation reactions. Oral P5P is dephosphorylated before absorption and reactivated in tissues; the active-form label does not establish superior outcomes.
Typical studied dose
Dossier trials include 10–40 mg/day for pregnancy nausea under obstetric care, up to 100 mg/day in older PMS studies, and 100 mg/day for about one month in the anxiety RCT. These are indication-specific trial exposures, not a safe daily range for everyone.

Forms and label traps
| Form on label | Active content | Studied range / day | Absorption & GI tolerance | Verdict |
|---|---|---|---|---|
| Pyridoxine hydrochloride: raw compound mass Best for: Comparison with the exact ingredient and dose basis on the Supplement Facts panel. |
82% elemental / active | 10–100 mg | Approximately 82% pyridoxine by mass for the stated HCl form. Use compound mode only for raw salt weight; a label already declaring Vitamin B6 mg must be entered as elemental to avoid double conversion. The 10–100 mg interval spans different therapeutic studies, not a general recommendation. See the form-specific clinical note; tolerance depends on dose and medical context. |

| Pyridoxal-5-phosphate: unspecified salt Best for: Comparison with the exact ingredient and dose basis on the Supplement Facts panel. |
Unstandardized: active content unknown | no comparable trial range | Oral P5P undergoes intestinal dephosphorylation. The salt/hydrate and B6-equivalent fraction are unspecified, so no raw-mass conversion or trial-matched range is established; consult the declared vitamin B6 amount. No proven safety exemption. Common label traps
- TRAP_01 // P5P cannot cause toxicity
No large clinical comparison demonstrates that P5P is risk-free. Form marketing cannot replace total-dose and duration review. - TRAP_02 // B6 repairs all neuropathy
Deficiency treatment and excess-induced neuropathy are different problems; high-dose self-treatment can worsen the latter. - TRAP_03 // Lower homocysteine means fewer heart attacks
B-vitamin biomarker reductions have not consistently reduced cardiovascular events or mortality.

- PREGNANCY NAUSEA Moderate · benefit
Reviews support symptom relief in mild-to-moderate early-pregnancy nausea; vomiting results are less clear. Obstetric therapeutic use, often with doxylamine, requires individual assessment.
PMID 36719452 PMID 27701665 PMID 42075106 - PMS Limited · benefit
Older low-quality trials suggest improvement in overall and depressive PMS symptoms; not established treatment for generalized anxiety.
PMID 10334745 PMID 2558186 - HOMOCYSTEINE VS EVENTS Limited · no benefit
B-vitamin combinations reduce homocysteine, but myocardial infarction and mortality benefits were not established; stroke findings depend on analysis and population.
PMID 28816346 PMID 25590290 PMID 37850302 - CARPAL TUNNEL Limited · no benefit
Small trials and reviews are inconsistent; no convincing clinical benefit established.
PMID 8724577 PMID 15098856 PMID 2749352 - DEPRESSION ASSOCIATION Limited · benefit
Higher dietary B6 intake is associated with lower depression risk; observational data do not prove supplementation treats depression.
PMID 33912967 - EXCESS-INDUCED NEUROPATHY Limited · makes it worse
Excess B6 can cause sensory neuropathy; in-vitro pyridoxine mechanisms cannot establish a precise safe threshold in humans.

Reader questions
How much is safe every day?
No universal toxicity threshold can be inferred. The US adult UL is 100 mg/day; the dossier flags a lower revised European limit needing verification. Sum all sources and avoid using a high-dose trial as a daily target.
Is P5P better than pyridoxine?
Both support B6 status. Oral P5P is processed before absorption, and large outcome and safety comparisons are missing.
Could tingling be from B6?
Yes. Excess-related neuropathy is a recognized concern; stop supplemental B6 and obtain assessment rather than escalating a nerve-support product.
Can B6 lower prolactin or treat pregnancy nausea on my own?
The prolactin claim lacks a solid RCT base in this dossier. Pregnancy nausea treatment belongs to obstetric care, with attention to medication, total intake and symptom severity.
Stack interactions
- Danger Levodopa without carbidopa [CAUTION // CLINICAL_REVIEW]
B6 can increase peripheral levodopa breakdown and weaken central effects; the dossier distinguishes levodopa plus carbidopa, which blocks this mechanism.
PMID 6087425 - Caution Isoniazid, cycloserine, hydralazine and penicillamine [CAUTION // CLINICAL_REVIEW]
These can impair B6 status; prevention or treatment of drug-related deficiency is clinician-directed.
PMID 6087425 - Caution Phenytoin / phenobarbital [CAUTION // CLINICAL_REVIEW]
Anticonvulsants can alter B6 status; high B6 doses can in turn alter some anticonvulsant concentrations.
PMID 6087425 - Caution B-complex, multivitamins and zinc/magnesium blends [CAUTION // CLINICAL_REVIEW]
Add the B6 declared in every product. Riboflavin supports B6 activation, but a mechanistic rationale does not justify megadosing.

Avoid or ask a doctor first if
Existing neuropathy, pregnancy, lactation, childhood, Parkinson treatment without carbidopa, or prolonged high intake require clinical review. Stop and seek assessment for tingling, numbness, touch sensitivity or gait imbalance; recovery from toxicity may take months.
